Diagnostic/Practice ACT Test — $50
Stand-alone testing for the new Enhanced ACT full-length test with answers and detailed written performance analysis. Given under the new enhanced ACT requirements for timing and section length. Included in Action, Growth, and Mastery learning plans.
CONSULTATION SERVICES
Consultation services are designed for individuals and families who need help understanding a situation, organizing information, evaluating options, and deciding what to do next.
These services are separate from Academic Coaching and Medical Companion/Advocacy Services.
30-minute focused consultation
A Clarity Call is appropriate when you have one specific question, concern, or decision and need help identifying the next step. This may include:
Talking through a school, family, work, or life concern
Clarifying what a document or process appears to require
Identifying questions to ask a professional
Deciding what information should be gathered
Determining whether a more comprehensive consultation is needed
A Clarity Call does not include advance document review or a written action plan.
75-minute initial consultation
This is the recommended starting point for a new or more complicated concern. Before the meeting, the client may submit a brief intake form and up to 10 pages of relevant documents. During the consultation, we will discuss the situation, identify priorities, consider available options, and develop practical next steps. The consultation includes:
Review of the client’s primary concerns
Identification of immediate and long-term priorities
Discussion of barriers, options, and available resources
Preparation of questions for relevant professionals
A brief written summary of recommended next steps
Additional document review may be added when needed.
60-minute consultation
Follow-up consultations are available after the Comprehensive Consultation or for established clients who need continued guidance. These sessions may be used to:
Review progress
Address new information
Adjust an existing plan
Prepare for an upcoming conversation or meeting
Work through a decision
Develop or refine practical systems
Brief notes and agreed-upon next steps are included.
Three 60-minute consultations
This package is appropriate when a concern is unlikely to be resolved in a single meeting. It may be useful for:
Parent coaching
Executive-function and household systems
College or career transitions
Independent-living preparation
Family decision making
Caregiving transitions
Workplace concerns
Ongoing organization and follow-through
Sessions may be used over a three-month period. The package saves $15 compared with booking three individual follow-up consultations.
Comprehensive consultation and customized written plan. The Systems Navigation Plan is intended for individuals or families managing several connected concerns at once. It includes:
A 75-minute consultation
Review of up to 20 pages of relevant documents
Identification of priorities, responsibilities, barriers, and deadlines
A customized written action plan
A list of questions for appropriate professionals
Recommended organizational tools or systems
One 30-minute follow-up session
Possible areas of focus include education, employment, independent living, family caregiving, major life transitions, or preparation for future planning.
This package helps a client prepare for an important school, workplace, family, legal, financial, or planning meeting. It includes:
Review of up to 15 pages of relevant documents
One 60-minute preparation session
Identification of the client’s primary concerns and desired outcomes
Development of a prioritized question list
Organization of notes and supporting information
A brief preparation summary to use during the meeting
Attendance at the meeting is not included and, when appropriate, may fall under a different service category.
This service helps clients organize and better understand complicated information. It may include:
Sorting and categorizing records
Developing a document index or timeline
Identifying missing information
Explaining unfamiliar terminology in everyday language
Highlighting areas that may require clarification
Preparing questions for an attorney, physician, educator, financial adviser, or other professional
This service does not include legal interpretation, medical advice, financial advice, diagnosis, or representation. A time estimate will be provided before extensive document work begins.
Additional consultation, research, document review, written planning, telephone calls, or correspondence beyond the amount included in a selected service is billed at:
$75 per hour, prorated in 15-minute increments
No additional billable work will be completed without the client’s knowledge and approval.
Need to Read Coaching provides coaching, consultation, organization, preparation, and systems-navigation support.
These services do not replace the work of an attorney, physician, mental-health professional, financial adviser, licensed social worker, educational evaluator, or other qualified professional.
When specialized guidance is needed, clients are encouraged to consult the appropriate professional. My role is to help clients understand the process, organize the information, prepare meaningful questions, and feel more confident about what comes next.
Medical Companion and Patient Support
Medical appointments can be stressful, especially when a patient is managing several conditions, seeing multiple providers, receiving unfamiliar information, or attending an appointment without a family member available to help.
Medical Companion and Advocacy Services provide practical, nonclinical support before, during, and after healthcare appointments. Services may include:
Organizing records and medication information
Helping the patient prepare questions
Accompanying the patient to an appointment
Taking detailed notes
Helping the patient communicate concerns
Clarifying what the patient heard during the visit
Organizing instructions and follow-up tasks
Helping family members understand what occurred
Creating a written visit summary and action list
Tracking referrals, tests, records, and future appointments
Need to Read Coaching does not diagnose medical conditions, recommend treatment, provide nursing care, make medical decisions, or replace the patient’s healthcare professionals.
Up to 45 minutes
Appointment Preparation is for patients who want to feel more organized and confident before attending a medical appointment independently or with a family member. This service may include:
Reviewing the purpose of the appointment
Organizing current concerns and symptoms
Preparing a medication list
Developing a prioritized question list
Identifying records or information to bring
Creating a simple appointment notes page
This service does not include appointment attendance or extensive medical-record review.
Up to two hours of appointment support
This service includes basic preparation, local travel, accompaniment during the appointment, note-taking, and a brief review immediately afterward. It may include:
A brief pre-appointment conversation
Review of the patient’s primary concerns
Preparation of important questions
Accompaniment to the medical appointment
Note-taking during the visit
Assistance making sure the patient’s questions are addressed
Review of instructions before leaving
A brief written summary and action list
The two-hour period begins at the agreed-upon meeting time and includes time spent waiting for the provider. Additional time is billed at $25 per half hour.
Preparation, appointment attendance, and written follow-up
This package is appropriate for new specialists, complicated appointments, multiple medical concerns, or situations involving several follow-up responsibilities. It includes:
A preparation session of up to 45 minutes
Review of up to 15 pages of relevant records
Preparation of a medication list and prioritized questions
Up to two hours of appointment accompaniment
Detailed notes during the visit
A written visit summary
A patient action list
A provider follow-up list
One brief follow-up call within seven days
Additional appointment time is billed at $25 per half hour.
The hourly rate may be more appropriate when:
Several appointments are scheduled on the same day
A procedure or appointment is expected to involve substantial waiting
The patient must visit more than one location
An appointment is likely to last longer than two hours
The patient needs assistance obtaining records or coordinating follow-up
The scope of the service cannot be predicted in advance
A time and cost estimate will be discussed before the service begins whenever possible.
This service helps patients and families create an organized medical-information system. It may include:
Developing a current medication list
Creating a medical history summary
Organizing provider contact information
Sorting laboratory reports and visit records
Creating a timeline of diagnoses, procedures, or hospitalizations
Tracking referrals and follow-up appointments
Preparing a medical binder or digital filing system
Identifying missing or outdated information
Preparing questions for medical professionals
Medical documents are organized and summarized for communication purposes. They are not independently interpreted or used to provide medical advice.
This service may help patients and families organize information during a hospitalization, rehabilitation stay, discharge, or transition home. Support may include:
Maintaining a record of providers and conversations
Organizing questions for the healthcare team
Taking notes during family or discharge meetings
Tracking medication and treatment changes
Organizing discharge instructions
Identifying follow-up responsibilities
Creating a calendar of appointments and deadlines
Helping family members communicate and divide responsibilities
This service does not include bedside care, transportation assistance, medication administration, patient lifting, personal care, or clinical monitoring.
With the patient’s permission, a concise written summary may be prepared for designated family members following an appointment. The summary may include:
The purpose of the visit
Major topics discussed
Instructions provided
Medication changes reported by the provider
Tests, referrals, or appointments ordered
Patient and family action steps
Matters requiring clarification
The summary documents what was communicated during the visit. It does not add medical opinions or treatment recommendations.
A Family Update Summary is already included in the Comprehensive Appointment Support package when requested.
Local travel within 15 miles of Need to Read Coaching at 104 Fairlane Drive is included in appointment packages.
For appointments beyond the local service area, the client will be informed of any additional mileage or travel charge before booking.
Parking fees, tolls, and other appointment-specific expenses are the responsibility of the client.
Transportation of clients is not included. Clients must arrange their own transportation unless a separate transportation policy and appropriate insurance coverage are established.
A limited number of reduced-rate appointments may be available for clients experiencing financial hardship.
Reduced-rate services are based on availability and may be limited to essential preparation, appointment accompaniment, and a basic written action list.
Clients may ask privately about availability. No detailed financial records are required.
Medical Companion and Advocacy Services are offered as private-pay, nonclinical support services.
Need to Read Coaching does not submit claims to Medicare, Medicaid, Medicare Advantage plans, or private health insurance. Clients should not assume that these fees will be reimbursed by an insurance plan.
Payment arrangements will be explained before services are provided. Clients will never be charged for additional work without their knowledge and approval.
Medical appointments often run late, and reasonable waiting time is included within the reserved appointment period.
Because appointment-companion services require travel and reserved time, cancellations should be made at least 24 hours in advance whenever possible.
Late-cancellation fees may be waived for hospitalization, sudden illness, severe weather, or other genuine emergencies.
If a healthcare provider cancels or reschedules after the companion has already traveled to the location, reasonable travel and time charges may apply.
Need to Read Coaching provides companionship, organization, communication support, documentation, preparation, and systems navigation.
I do not:
Diagnose medical conditions
Interpret test results as a healthcare professional
Recommend or change medications
Recommend treatment
Provide nursing or personal-care services
Make healthcare decisions for a patient
Sign medical or legal documents on a patient’s behalf
Guarantee access to treatment, insurance coverage, or a particular outcome
Replace physicians, nurses, social workers, attorneys, case managers, or other qualified professionals
The patient remains responsible for medical decisions and should direct all clinical questions to the appropriate healthcare professional.
My role is to help the patient arrive prepared, leave informed, and understand what needs to happen next.
